Curekey Telehealth Consent

Effective Date: September 16, 2025

Telehealth uses electronic communications to enable licensed healthcare providers at different locations to evaluate, diagnose, treat, and educate patients. Curekey LLC (“Curekey,” “we,” “us”) provides technology and non‑clinical services that allow you to access independent medical groups and licensed clinicians (“Medical Groups” and “Providers”) and to coordinate prescription fulfillment with independent pharmacies (“Pharmacies”). Curekey is not a medical group or healthcare provider.By checking the consent box, creating an account, or using telehealth features, you acknowledge that you have read, understand, and consent to receive care via telehealth, subject to applicable state law and the Medical Group’s policies.

1) Services & Modalities

Telehealth services may include:

  • Review of medical records, history, questionnaires, images, and test results (store‑and‑forward/asynchronous communication)
  • Live two‑way audio and/or video encounters
  • Interactive audio with store‑and‑forward components
  • Electronic prescribing and care coordination with Pharmacies
  • Patient education and remote monitoring data (if applicable)

Electronic systems used for telehealth incorporate administrative, physical, and technical safeguards to protect the confidentiality and integrity of your information. However, no system is 100% secure, and risks remain (see Section 3).

2) Relationship to Your Primary Care Clinician

Telehealth services accessed through Curekey are in addition to, not a replacement for, your primary care clinician. Responsibility for your overall medical care should remain with your local primary care clinician, if you have one. Providers will determine clinical appropriateness for telehealth on a case‑by‑case basis.

3) Expected Benefits & Possible Risks

Benefits may include:

  • Improved access to licensed Providers without traveling to a clinic
  • Faster review of labs/images and the ability to consult specialists when appropriate

Possible risks include (not exhaustive):

  • Technical failures (e.g., poor connection, platform outage) that may delay care and require rescheduling or in‑person follow‑up
  • Insufficient information quality from transmitted data leading to the need for additional evaluation
  • Privacy/security incidents despite safeguards, including unauthorized access or disclosure due to failures of equipment, software, or network security
  • Incomplete records that could increase the risk of medication interactions, allergic reactions, or judgment errors

If you experience an adverse reaction, need follow‑up, or cannot communicate due to a technical issue, contact support@curekey.com so we can facilitate outreach from the Medical Group. For emergencies, call 911 immediately.

4) Your Consent & Acknowledgments

By consenting, you acknowledge and agree that:

  1. Consent to Telehealth. You consent to receive medical services via telehealth technologies. You understand telehealth may involve the electronic transmission of personal medical information to Providers who may be in other locations (including out of state) as permitted by law.
  2. No Guarantee of Prescription. A telehealth consultation does not guarantee that a prescription will be written. Controlled substances, including narcotic pain medications, will not be prescribed.
  3. Alternatives. Alternatives to telehealth (e.g., in‑person care) are available to you. Certain testing (e.g., labs) may be performed at a facility or at your location at the Provider’s direction.
  4. Right to Withdraw. You may withhold or withdraw consent to telehealth at any time without affecting your right to future care or treatment. You may suspend or terminate use of telehealth services at any time.
  5. Privacy & Security. Federal and state laws require protection of your health information. Curekey and its partners take steps to safeguard your information, but email and SMS may be unencrypted and could be intercepted. By providing your email/phone or initiating email/SMS communication, you accept these transmission risks for non‑emergency communications.
  6. Technical Issues. You agree to hold harmless Curekey for delays in evaluation or information loss due to technical failures outside our reasonable control. Providers may determine a telehealth visit is not clinically appropriate and may recommend an in‑person visit.
  7. Care Team Participation. Personnel other than your Provider (e.g., clinical staff, technical staff) may be present to operate telehealth technologies. You will be informed of their presence and may: (i) request to omit sensitive history details, (ii) ask non‑medical personnel to leave the session, or (iii) end the session at any time.
  8. Access to Records. You may request a copy of your medical records from the Medical Group/Provider. Reasonable fees may apply as permitted by law.
  9. Identity & Location Attestation. You certify that the information you provide is accurate, that you are 18 years or older, and that you will accurately disclose your physical location at the time of each telehealth encounter, as required by state law and for licensing purposes.
  10. Photos/Media for Care. You consent to submitting photographs/videos (e.g., scalp/hairline) and understand they become part of your medical record.
  11. Pharmacy Disclosures. If a prescription is issued, you authorize the sharing of necessary information with your chosen Pharmacy(ies). Compounded medications may be prescribed; compounded drugs are not reviewed or approved by the FDA and may carry unique risks.
  12. No Emergency Care. Telehealth through Curekey is not appropriate for emergencies. If you believe you are experiencing an emergency, call 911.

5) State‑Specific Notices

Telehealth availability and requirements vary by state (e.g., identification, modality, prescriber‑patient relationship, consent form content). Your Provider will comply with applicable state law at the time of service and may provide additional disclosures during intake.

Prescription products require an online consultation with a licensed healthcare provider who will determine if a prescription is appropriate. Curekey does not offer or ship medications in jurisdictions where it is not licensed. Compounded medications are prepared by partner pharmacies;compounded products have not been reviewed by the FDA for safety or efficacy. If you experience a medical emergency or severe adverse reaction, call local emergency services immediately.
